Treatment of Lung Mesothelioma

Surgery.

Used three types of surgery in the treatment of mesoepitelioma. Diagnostic surgery to obtain a sample for biopsy is performed by thoracoscopy (*), by laparoscopy or surgery or pleural mediatinoscopia open. Secondly, palliative surgery include partial pleurectomy pleurodesis, thoracoscopy with pleurodesis and more rarely, pleuroperitoneal shunt. The third is curative surgery involving extrapleural pneumonectomy in order to remove most of the tumor. In most centers, surgery is followed by adjuvant therapy. Not all patients are suitable for this procedure, which otherwise has a 6% mortality

Chemotherapy

Although there have been several chemotherapy regimens, none has proven to be curative. The two most useful (to the effect of reducing the tumor mass and improve the symptoms) are cisplatin + pemetrexed + gemcitabine and cistaplatino. In both cases, dexamethasone is recommended to add folic acid supplements and vitamin B12

Radiotherapy

Radiation therapy has been used for many years with disappointing results due to the diffuse nature of the tumor as it may cause radiation pneumonitis. Only one has been found effective when applied locally in the scar area after surgery to avoid the spread of tumor cells remains

Other treatments under investigation include immunotherapy, gene therapy, photodynamic therapy and some anti-angiogenic agents

Recent reports Noted anti-folate That many chemotherapeutics HAD good response rates, toxicity Was But the too great. For example, methotrexate Had a 37% response rate. However, 58% grade 4 toxicities Were Noted Including one death related Treatment.

what is mesotheliomaThe mesothelioma or mesothelioma, also known as “Malignant mesothelioma” or “Cancer mesothelioma, is a rare form of cancer that develops in a protective membrane that covers many of the internal organs of the body, or mesothelium mesothelium. It is usually caused by exposure to asbestos or asbestos.

Its most common site is the pleura (the outer sheath of the lungs and internal chest wall), but it may also occur in the peritoneum (the sheath of the abdominal cavity), the heart or the pericardium (a sac that surrounds the heart .) Diffuse Peritoneal Mesothelioma is a rare disease. There is no epidemiological evidence about the cause except a small percentage of patients who are associated exposure to asbestos. There is no predominance in sex, age or race. Not a member no genetic association, dietary preference or geographical distribution. Usually occurs sporadically without family distribution.

Mesothelioma is unusual and may have a wide spectrum with different biological aggressiveness. The cystic variant can cause repeated episodes of abdominal pain but usually does not cause death of the patient. By contrast, the most aggressive forms may present in mesenteric lymph node metastases at the time of diagnosis. The number of mitosis, cell morphology and histological types determine the aggressiveness of the disease. But you can expect different types or levels of tumor mestotelial in the same patient.

Clinically, the diffuse dissemination of malignant mesothelial cells determines a copious production of liquid, so that all patients with peritoneal mesothelioma present with large amount of ascites at initial diagnosis.
